Medical Questionnaire Form

The club has setup a process to provide patient information to paramedics in the event of a medical emergency.

This is a purely voluntary process.  If you choose to have your medical information on file, please complete the questionnaire, put it in a sealed envelope with your name clearly written on the front and the year.  Put it into the Vice President slot in the office area.  It will then be kept in an accordion file, which will be located next to the AED/medical cabinet in the clubhouse.

Blank questionnaires and envelopes can be found on the card table in the sun room, and on the website in the Members Area under Health and Safety.
